If you are having problems completing the Questionnaire online, you can download softcopies of the Questionnaire (Microsoft Word format). Please click on the links below.

You only have to complete Sections I to IV once; however, please complete a separate Project/Activity Information form for each ICT-related UNDP Country Office project or activity.

NOTE: To mark YES/NO checkboxes, just double click on the checkbox and pop-up window will appear. Under the "Default value" option, select "Not checked" or "Checked", then click on "OK".
